Buffalo Pile is a map location and dungeon. Inside the pile is a circular series of 5 rooms. The right doorway leads to the next room in the sequence, eventually wrapping around back to the first room. The left doorway in each room (initially) leads to a major fight; once that is complete, the left doors connect to the preceding room in the circular sequence.
Room 1 (Entryway)[ | ]
You can inspect the cards on the table. Each has a brief randomized description.
- If you freed Grutch, the cards are ripped up and may not be inspected.
Room 2 (Vat Room)[ | ]
On a table you can find oversized chemical beakers. Against the back wall are 4 vats of liquid (left to right): blood, milk, ichor, and lemonade.
You can use the vats to fill your beaker with combinations of liquids. You can find clues for valid combinations by eavesdropping on cultists in room 5. There are, however, only 3 valid mixtures, as follows:
- beaker of buffalo reanimating fluid - Milk then blood then ichor.
- beaker of lemonade (potion +15 to 3 M stats) - Lemonade three times.
- beaker of fudge (food +23 to 3 M stats) - Milk then milk then lemonade.
Note that order is important; for example, using lemonade, milk, milk will result in a ruined concoction instead of fudge.
You can acquire a new beaker once you've used one. However, note that the 3rd time you add something to a beaker you destroy the last tank accessed (for the rest of that playthrough). So ideally you would make a beaker of buffalo reanimating fluid (destroying the ichor tank), and then your choice between fudge and lemonade (as either will destroy the lemonade tank).

On the laboratory table you can assemble Buffalo Buffalo Buffalo Bill. For this you need the buffalo soldier bones, and the beaker of buffalo reanimating fluid. The bones are acquired here (on the shelf), but first require you get the buffalo soldier diagram from the footlocker in room 5, for which you will need the Buffalo Pile locker key found here in a robe hanging on a rack (you could also use Lockpickin' 3 and a needle). The beaker of buffalo reanimating fluid is manufactured in room 2.
The holding cells contain an infinite supply of combats for your grinding pleasure. Other items can be found by searching the shelf full of crates of bones, human and buffalo:
- If you have the quest from the Petting Cemetery you can grab a box of buffalo bones.
- If you have the buffalo soldier diagram you can acquire buffalo soldier bones.
- If you have the Unlimited Bones perk, you can acquire some bones:
- Up to 10 skeleton bones, depending on how many you already have.
- Up to 5 skull chips (if you have the Raise Skeletal Thrall perk), depending on how many you already have.
- Up to 5 handfuls of loose teeth (if you have the Raise Skeletal Gunslinger perk), depending on how many you already have.
Room 4 (Bathroom)[ | ]
Two toilets can be found, a good one and a bad one; both may be flushed.
- A pile of magazines yields a class-specific skill book, one of Advanced Beancraft, Advanced Cow Punchin' or Modern Snake Oiler.
- A pile of trash contains a clue for your Necromancer Journal.
Room 5 (Sleeping Quarters)[ | ]
Hiding in the wardrobe allows you to gather clues for using the vats in Room 3 (if you have not yet killed the guards at least).
- blood after ichor, fail and turn to liquor,
- milk after ichor might as well reanimate wicker,
- milk before ichor, soldier wakes up quicker,
- blood before milk as obsolete as the word thilk,
- ichor after milk rituals smooth as silk,
- ichor after blood way to go stud,
- ichor before blood bad idea bud,
- milk after blood rituals a dud.
A few additional items can also be acquired.
- On a pedestal you can find a Nex-Mex Book.
- A locked footlocker can be opened (with Lockpickin' and a needle) to reveal a black hood, a bottle of buffalo tranquilizers, and 50-80 meat.
- Another footlocker has a "serious" padlock on it. You can unlock it with the Buffalo Pile locker key, or Lockpickin' 3 and a needle. Inside is the buffalo soldier diagram and some random grave loot.

The guard fight is accessed by choosing the left door from any room. You will fight 2 cultist mages, 3 Buffalo Soldiers, 3 Buffalo Soldier rifleman, and Doug.
- If you choose to recruit Buffalo Buffalo Buffalo Bill, he will end up "taking care of them".
- You will still receive the Unlimited Bones perk.

The formula for Beaker of fudge is a reference to a children's rhyme: "Milk, milk, lemonade, 'round the corner fudge is made".
